Job Summary

Mass General Brigham is seeking a skilled Registered Nurse to fill a Case Management position. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring timely and effective treatment outcomes, and promoting quality and cost-effective care.

Key Responsibilities
  • Assess patient health and psychosocial needs, developing a comprehensive case management plan to maximize healthcare responses, quality, and cost-effective outcomes.
  • Collaborate with the interdisciplinary team, patient, and family to coordinate healthcare services, minimize fragmentation, and promote optimal outcomes.
  • Provide education and support to patients and families, empowering them to make informed decisions and take an active role in their care.
  • Ensure appropriate use of healthcare services, allocating resources to improve quality of care and maintain a balance with cost-effective care.
  • Develop and implement discharge plans, ensuring a safe transition to the next level of care and promoting continuity of care.
Requirements
  • Bachelor's degree in a related field; Master's degree preferred.
  • 3-5 years of experience in case management, utilization review, or discharge planning, preferably in an acute, rehabilitation, skilled nursing, or outpatient setting.
  • Current, unrestricted license to practice in Massachusetts in a health or human services discipline.
  • Knowledge of community services, healthcare systems, and criteria for levels of care.
  • Computer proficiency, with Microsoft Office applications preferred.
  • Certification in Case Management preferred.
