About the Position

Lane County Behavioral Health is seeking a skilled Community Health Nurse 2 to join our team in a clinic-based setting. This exciting opportunity provides support work to individuals experiencing severe and persistent mental illness, working in collaboration with our child, adolescent, and adult mental health services teams.

Key Responsibilities:
  • Provide nurse case management and care management for individuals with severe and persistent mental illness, including co-morbid medical conditions.
  • Triage and service coordination with clinical and medical staff, primary care providers, and allied community agencies, including crisis intervention as required.
  • Health and wellness promotion through identification of mental health issues and intervention/prevention of health problems, skills training, and patient and family psycho-education.
  • Demonstrate competency in consulting with primary care providers, psychiatry, emergency services, and other services on behalf of mutual patients.
About the Division

Lane County Behavioral Health provides services to over 1,600 individuals with behavioral health conditions, working with community partners to provide a continuum of care from prevention through treatment and aftercare in an environment that promotes a culture of resilience and recovery.

Requirements:
  • Bachelor of Science in Nursing degree from an accredited college or university.
  • Oregon Health Licensure required: Oregon Nurses License.
  • NPI.
  • Transcripts.
  • Diploma.
  • CV or Resume.
  • Oregon Driver's License.
  • Immunization records/TB within 10 days of employment.
