Job SummaryWe are seeking a Physician to join our team and work as a lead in our team-based care environment. As a Physician with Bloom, you will have the opportunity to provide high-quality care to our patients and be part of a value-based care model that focuses on quality of care and better outcomes.
Responsibilities- Deliver comprehensive primary care at home visits to patients
- Manage the health outcomes of an empaneled patient population
- Coordinate patient care with a team of Advanced Practice Professionals
- Engage with and utilize population health data to drive superior patient outcomes
- Attend ongoing educational opportunities
- Base Salary of $225,000
- Quality Bonus of up to $35,000
- Participation in our employee equity plan
- Flexible schedules (3,4,5-day work weeks)
- 10 visits per day
