Our team consists of two physicians, four APPs, and a psychologist providing on-site geriatric behavioral health support. We cater to seniors with multiple chronic conditions, with the goal of helping patients achieve their health goals.
As a member of the independent practice association (IPA), PHPprime, we have access to wrap-around services that support our practice's success, including care coordination, practice transformation coaching, and population health management.
The Community:Located in Colorado Springs, CO, our community offers a great quality of life, with easy access to outdoor recreation and entertainment venues. This region is recognized nationally as one of the top places to raise a family and live a healthy lifestyle.
The Position:- Ambulatory primary care for older adults (60 and up)
- Innovative patient-centered care
- Collaborative open workspace
- No hospital or facility responsibility
- Office hours, Monday through Friday, 8:00 am to 4:30 pm
- 7 to 10 patients per day
- Dedicated MA for each provider
- One RN who manages transitions of care (hospital, ED, facilities)
- Night and weekend call-shared with other physicians (minimal calls)
MD or DO degree from a fully-accredited medical or osteopathic school of medicine
Residency trained in internal or family medicine
Excellent communication and interpersonal skills
Benefits:Compensation and benefits package for a full-time physician
The incoming physician will join AgeWell as an employee and have the opportunity to become a partner in the practice after one year
Base salary of $250,000 for the first year
5 days CME up to $2,500 per year
PTO 4 weeks
