Adult Home Health Team Member

We are seeking a skilled and compassionate Registered Nurse to join our Adult Home Health team. As a Community Health Nurse, you will deliver high-quality care to patients in their homes, providing a wide range of skilled nursing services while working to the top of your licensure.

Our team is comprised of experienced professionals who collaborate to support patients in remaining living in their homes. We value autonomy, flexibility, and meaningful relationships with our patients.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements:

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired but not required.
  • Graduate nurses considered for our Community Health Nurse I level.
