Adult Home Health Team

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Adult Home Health team. As a Community Health Nurse, you will deliver high-quality care to patients in their homes, providing a wide range of skilled nursing services while working to the top of your licensure.

Our team is comprised of experienced professionals who collaborate to support patients in remaining living in their preferred setting. We value autonomy, flexibility, and meaningful relationships with our patients.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments and develop individualized care plans to meet patients' unique needs.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.

Requirements:

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired but not required; we are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
  • Graduate nurses are considered for our Community Health Nurse I level.
