Adult Home Health Team Member

We are seeking a skilled and compassionate Registered Nurse to join our Adult Home Health team. As a Community Health Nurse, you will deliver high-quality care to patients in their homes, providing a wide range of skilled nursing services while working to the top of your licensure.

Our team is comprised of experienced professionals who are dedicated to supporting patients in their homes. We offer a comprehensive and collaborative approach to care, with a focus on building meaningful relationships with our patients and their families.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' needs and develop individualized care plans.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes.

Requirements:

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired but not required.
  • Graduate nurses are considered for our Community Health Nurse I level.
