Job Summary:

Are you a compassionate and skilled nurse looking for a challenging and rewarding role in home health care? We are seeking a part-time Community Health Nurse to join our Adult Home Health team. As a member of our team, you will have the opportunity to provide high-quality, patient-centered care to individuals in their own homes, working collaboratively with a multidisciplinary team to achieve optimal health outcomes.

Responsibilities:

• Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care.
• Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
• Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal health outcomes.
• Participate in ongoing education and training to stay current on best practices in home health care and to enhance your skills and knowledge.

Requirements:

• Greater than two years of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired.
• Previous home care experience desired but not required.
• Graduate nurses are considered for our Community Health Nurse I level.
