Our team of experienced professionals works closely with clients to navigate and secure the necessary services to live independently in their community.
Job Summary:
The Case Manager/Social Worker will provide case management activities necessary to meet the needs of clients assigned to the consolidated care team. This role involves developing individualized care management plans, implementing care plans through brokering and coordinating services, and monitoring and evaluating clinical outcomes to ensure services are provided in a professional, comprehensive, and cost-effective manner.
Key Responsibilities:
- Complete assessments to identify client needs and desires for service delivery.
- Develop and modify Person-Centered Service Plans annually or as needed.
- Ensure participant choice of providers and ensure Behavioral Support and Crisis Plans are consistent with the Individual Support Plan.
- Report incidents of participant abuse, neglect, or exploitation immediately.
- Actively coordinate with other individuals and entities essential in physical and behavioral care delivery.
- Research and develop alternative solutions to client needs.
- Ensure confidentiality and maintain proficiency in company-sponsored training and certifications.
- Attend required trainings and recertification classes.
- 50% travel to participants as needed.
Qualifications:
- BS/BA degree in related coursework.
- Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
- Experience with Microsoft Office and state-maintained databases.
- Maintain CPR/First Aid certification and updated state-related Clearances.
- Valid Driver's License with personal vehicle insurance coverage and current vehicle registration.
