Job Summary

We are seeking a highly motivated and compassionate Resident Care Coordinator to join our team at Elk Ridge Health & Rehabilitation Center. As a key member of our assisted living community, you will play a vital role in ensuring the highest level of quality care and support for our residents.

Key Responsibilities
  • Resident Care Duties:
    • Provide personalized assistance with activities of daily living (ADLs) such as grooming, bathing, and dressing, as requested by residents or directed by supervisor.
    • Attend to the personal care needs of residents in a timely and professional manner.
    • Regularly review, follow, and adhere to residents' service plans to ensure optimal care and support.
    • Document and report care provided and resident observations to maintain accurate records.
    • Perform vital signs and heights and weights as required.
    • Respond promptly to emergency calls and ensure the safety and well-being of residents.
    • Monitor residents' health and report any changes or concerns to supervisor.
    • Demonstrate knowledge of infection control and universal precautions to maintain a safe environment.
    • Provide information, encouragement, and participate with residents in activity-focused programs to promote engagement and socialization.
    • May be asked to provide assistance to residents in Assisted Living and/or Memory Care.
  • Relationship Duties:
    • Develop and maintain positive relationships with residents, families, and colleagues through effective communication and empathy.
    • Demonstrate a passion for caring and a commitment to delivering exceptional service.
    • Relate to and work with residents, families, and colleagues who may be ill, disabled, elderly, emotionally upset, or hostile.
  • Department Duties:
    • Attend and participate in regular staff meetings to address resident, departmental, and community needs.
    • Responsible for maintaining a clean and safe environment, including common areas, occupied and vacant apartments, and performing laundry services in accordance with community policies and procedures.
    • Meet the general health requirements set forth by the policies of Elk Ridge Health & Rehabilitation Center.
    • Perform all other duties as assigned by supervisor.
    Requirements
    • Effectively communicate in the English language and follow written and verbal instructions.
    • High School diploma or GED (preferred).
    • 6-months experience in a senior living community (preferred).
    • Valid Adult Basic First Aid and CPR Certification (preferred).
    Work Environment

Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

Must be able to push, pull, move, and/or lift a minimum of 25# to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

May be necessary to assist in the evacuation of residents during emergency situations.
