Home Health LPN Job Description

We are seeking a skilled and compassionate Home Health LPN to join our team at Southwest Medical, a part of OptumCare. As a Home Health LPN, you will provide direct care to patients in their homes, working under the leadership of a Registered Nurse.

Key Responsibilities:

  • Provide direct care to patients, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Assess patient needs and develop individualized care plans to ensure positive outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and maintain accurate records.
  • Comply with state and federal regulations, including HIPAA and OSHA guidelines.

Requirements:

  • Current, unrestricted NV LPN license or temporary NV LPN license with the ability to obtain permanent NV LPN license upon expiration of temporary NV LPN license.
  • IV Certification.
  • 1+ years of demonstrated competence in acute care, critical care, oncology, HIV/AIDS, advanced illness, or medical/surgical patient care settings.
  • Proven ability to lift up to 50 lbs and perform physical demands of the job.
  • Access to reliable transportation for patient visits.

Preferred Qualifications:

  • Home Health experience.
  • Knowledge and competency in wound care and use of wound vacs.
