Primary Care Provider Manager

Responsibilities:
- Participate in the medical evaluation of patients admitted to the Skilled Nursing Facility
- Evaluate and document the medical necessity for specific levels of care, facilitating transfers if necessary
- Provide advice and information to staff and referring physicians on medical interventions consistent with the medical plan of care
- Consult with attending physicians and referral sources to ensure comprehensive and efficient medical care for patients
- Communicate with patients and families regarding their medical condition and plan of care
- Participate in the review and development of practice protocols within the facility
- Inform primary physicians and referral sources of the outcomes of Nurse Practitioner consultations and evaluations
- Document interactions between nursing staff, referral sources, patients, and families
- Maintain regular contact with assigned referral sources
- Complete, maintain, and submit accurate clinical notes regarding patients' conditions and care
- Utilize electronic medical records to ensure quality care, accuracy, and effective communication with the Skilled Nursing Facility
- Provide education and information to the SNF care team and/or medical community as needed
- Participate in resolving interpersonal conflicts and clinical or ethical concerns
- Engage in research activities or special projects as assigned
- Participate in the call schedule with other members of the call team (compensated separately)
- Perform other duties as assigned

Requirements:
- Experience: Minimum of two (2) years of professional experience in healthcare
- Education and Training: Nurse Practitioner or Physician Assistant licensure required
- License, Certification & Registration: Possess and maintain current CPR Certification, Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant, Valid driver's license
- Other Requirements: Demonstrates excellent verbal and written communication, and organizational skills, Strong observation and problem-solving skills, Experience working with an Interdisciplinary Team preferred
