Job Summary

The successful candidate will provide exceptional primary care services to patients of all ages, promoting health and well-being in our community. They will work collaboratively with our healthcare team to deliver high-quality patient care services, develop and implement effective patient care plans, and maintain accurate and comprehensive medical records.

In this role, you will have the opportunity to work with diverse patient populations, participate in training and education programs, and contribute to the growth and development of CHP.

Key Responsibilities
  • Provide advanced primary and specialty nursing services to patients of all ages.
  • Develop and implement effective patient care plans.
  • Maintain accurate and comprehensive medical records.
  • Collaborate with healthcare professionals and families to promote healthy habits and disease prevention.
Requirements
  • Master's degree from an accredited nursing program.
  • Licensure in the State of Massachusetts and board-certification by either ANCC or AANP.
  • Prescriptive authorization.
  • Experience providing primary care as a Family Nurse Practitioner.
  • Knowledge of computer-based data management programs and medical records technology.
  • A sound understanding of all pertinent federal and state regulations.
  • Excellent verbal and written communication skills.
Salary and Benefits

The estimated salary range for this position is $104,500 - $120,000 per year, depending on experience and qualifications. We offer a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
