Job Summary

The Clinical Care Coordinator is a key member of our team responsible for facilitating care management. This role provides clinical support through daily prioritization of patient services and follow-up with referral sources, primary care providers, patients, clinicians, and clinical leaders.

We are seeking an experienced professional with at least two years of community health experience in a Medicare-certified agency. A strong understanding of home care principles and practices is essential.

The ideal candidate will have demonstrated coaching/team building skills, excellent interpersonal skills, and the ability to handle conflict effectively. Flexibility in thought process and time availability is also crucial.

The successful candidate will possess an Associate's Degree in Nursing as a minimum requirement, with a Bachelor's Degree in Nursing being preferred. A current RN license in Massachusetts is mandatory, along with CPR certification.

This position requires the ability to exercise independent judgment, maintain confidentiality, and demonstrate problem-solving skills. The candidate should be able to coach, mentor, and motivate others while adhering to customer service standards. The ability to work flexible hours and possess a valid driving license is also necessary.

We estimate the salary for this role to be around $75,000 - $90,000 annually, depending on experience.
