Job Summary:
Summa Health System is seeking a skilled Social Worker to join our team. As a Transitional Care Manager, you will collaborate with ambulatory and inpatient care management teams to assess, plan, implement, coordinate, monitor, and evaluate services for patients with multiple health needs.
Key Responsibilities:
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Assess patient needs and develop strategies to promote quality, cost-effective outcomes
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
- Maintain accurate and confidential patient records
- Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
- Masters degree in Social Work (MSW) or related field
- Licensed Independent Social Worker (LISW) in the State of Ohio
- 3+ years of social work experience, preferably in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
