Social Worker II

We are seeking a highly skilled Palliative Care Social Worker II to join our team at Akron Children's Hospital. As a member of our multidisciplinary team, you will work collaboratively to address the complex needs of patients and their families.
Key Responsibilities:- Conduct comprehensive psychosocial assessments of patients and their families to identify needs and develop effective plans.
- Collaborate with healthcare teams to provide education, training, and consultation on psychosocial issues that impact patient care.
- Provide case management services to patients and their families, focusing on social determinants of health and community resources.
- Develop and implement programs to promote patient and family well-being, including support groups and educational workshops.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Master's degree in Social Work from an accredited program.
- Licensed Social Worker (LSW) or equivalent.
- 1-3 years of experience in healthcare, preferably in palliative care or a related field.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
