Position Summary

The Engagement and Enrollment Specialist plays a crucial role in facilitating outreach and enrollment for individuals eligible for Care Coordination services. This position is vital for both Medicaid and Non-Medicaid recipients who are managing co-occurring medical, mental health, and/or substance use challenges.

The specialist is responsible for conducting outreach through various channels—telephonic, written, and in-person—to engage and retain members eligible for Health Home services. Collaborating closely with the Care Coordination team, the specialist identifies, locates, and enrolls eligible individuals while providing essential information about community resources that support wellness objectives.

Key Responsibilities:
  • Deliver comprehensive engagement and enrollment services to facilitate Health Home enrollment and retention for eligible individuals.
  • Leverage internet databases and health record systems to ascertain individual eligibility and contact details.
  • Maintain accurate clinical documentation in compliance with HIPAA regulations, ensuring confidentiality for all interactions.
  • Assist Care Coordinators in identifying and securing necessary referrals to community and network providers through various outreach methods.
  • Conduct follow-up services to coordinate reminders, medication checks, and wellness assessments as requested by the Care Coordination team.
  • Participate in collaborative meetings and member transports with Care Coordination staff.
  • Provide timely updates regarding member status, progress, and needs to Care Coordinators and relevant stakeholders.
  • Complete all necessary assessments for enrolled members upon their entry into the program.
  • Engage in community meetings and case conferences as required.
  • Fulfill additional duties as assigned.
Qualifications

Essential Skills and Experience:
  • Bachelor's degree with a minimum of two years of experience working with individuals facing mental health and substance use challenges.
  • Proficiency in electronic record systems is highly desirable.
  • Ability to travel using public transportation and effectively manage time to meet program and regulatory standards.
  • Experience in outreach, client engagement, assessment, direct service, and documentation is required.
