Job Title: Patient Care Coordinator

Regional One Health is seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care, ensuring seamless transitions, and promoting a culture of excellence in patient-centered care.

Key Responsibilities:
  • Coordinate patient care activities, including assignments, scheduling, and communication with healthcare teams.
  • Develop and implement patient care plans, ensuring alignment with hospital policies and procedures.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care, including medication management, wound care, and pain management.
  • Provide education and support to patients and families, promoting health literacy and self-management skills.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing evidence-based practices.
  • Maintain accurate and timely documentation, ensuring compliance with regulatory requirements and hospital policies.
Requirements:
  • Graduate of an approved nursing program.
  • Current RN licensure in the state of Tennessee.
  • Minimum 1 year of clinical experience, preferably in a hospital setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to prioritize tasks, manage multiple responsibilities, and maintain a high level of productivity.
