Job Title: Full-Time Registered Nurse

Trinity Health is seeking a highly skilled and compassionate Full-Time Registered Nurse to join our team. As a Primary Care Nurse, you will provide high-quality patient care to individuals in their homes, working collaboratively with our healthcare team to achieve optimal outcomes.

Key Responsibilities:
  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Develop and implement individualized care plans in collaboration with our Population Health Case Manager.
  • Supervise and support Home Healthcare Aides and LPNs as needed, ensuring seamless care delivery.
  • Utilize motivational interviewing and health coaching techniques to promote patient engagement and desired outcomes.
  • Manage patient care activities, including scheduling, documentation, and communication with healthcare teams.
Requirements:
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one year professional nursing experience, preferably in Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit.
  • Strong computer and technology skills required.
  • Current Driver's license and reliable transportation.
