Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our Home Care team. As a Home Care RN, you will provide primary nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care.

Responsibilities
  • Provide one-to-one care to patients in their homes, assessing their needs and developing individualized care plans.
  • Collaborate with our Population Health Case Manager to ensure seamless care coordination and achieve top decile client outcomes.
  • Supervise and support Home Healthcare Aides and LPNs as needed, promoting a culture of teamwork and excellence.
  • Utilize advanced technology to enhance patient care and outcomes, including our integrated virtual care program, Home Care Connect.
Requirements
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one year professional nursing experience, with home healthcare experience preferred.
  • Strong computer and technology skills, with ability to learn and adapt to new systems.
  • Current Driver's license and reliable transportation.
