Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care team. As a Primary Care Nurse, you will provide high-quality patient care to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide primary nursing care to patients in their homes, following physician orders and organizational policies.
  • Collaborate with Population Health Case Managers to develop and implement care plans.
  • Supervise and support Home Healthcare Aides and LPNs as needed.
  • Provide therapeutic interventions and achieve top decile client outcomes and patient satisfaction.
  • Utilize educational tools and pathways to optimize patient care.
  • Participate in quality improvement activities to enhance agency performance.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one year professional nursing experience, preferably in Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit.
  • Strong computer and technology skills required.
  • Current Driver's license and reliable transportation.
