Key Responsibilities- Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
- Develop and implement individualized care plans, working closely with Population Health Case Managers to ensure comprehensive care.
- Supervise and support Home Healthcare Aides and LPNs as needed, promoting a collaborative and patient-centered approach to care.
- Utilize motivational interviewing and health coaching techniques to empower patients to achieve their health goals.
- Manage patient schedules and visit utilization to optimize agency performance and achieve high-quality care.
- Graduate of an approved Nursing education program, BSN preferred.
- Current Registered Nurse licensure in the state of practice.
- Minimum of one year professional nursing experience, with Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit experience.
- Strong computer and technology skills required.
- Current Driver's license and reliable transportation.
