Your Role

The Advanced Social Services Coordinator offers telephonic support and resource referrals to Blue Shield Promise members, while also providing administrative assistance to the Social Service Behavioral Health Care Management Team and their supervisors.

The Coordinator frequently engages with members and their families to assess social service and behavioral health requirements, aiming to enhance the quality of care provided to members.

This role involves collaboration with team members and various internal departments, serving as the primary telephonic contact for incoming inquiries to the Social Services department.

Your Responsibilities

In this position, you will:

1. Establish trusting relationships with members, delivering person-centered support and services.

2. Conduct telephonic assessments for behavioral health and social determinants of health needs for members with mild to moderate acuity, addressing social barriers and care gaps.

3. Facilitate care coordination, referrals, and connections to Health Plan resources and community organizations for members and their families.

4. Accurately document screenings, assessments, referrals, and case notes in the electronic case management systems in accordance with departmental guidelines.

5. Act as a mandated reporter, filing and following up on reports to the appropriate government agencies (e.g., Adult Protective Services, Child Protective Services, etc.).

6. Provide telephonic support to members and their families post-discharge from hospitals and other medical facilities.

7. Coordinate services with state and community programs on behalf of members, especially when they are unable to do so independently.

8. Prepare and complete logs and reports as necessary for meetings and audits.

9. Perform other duties as assigned.

Your Knowledge and Experience

- High School Diploma required.
- Bachelor's Degree in Social Work or equivalent preferred.
- Requires comprehensive knowledge of operational systems and practices typically acquired through extensive experience and/or education.
- Typically requires a minimum of 5 years of experience; vocational or technical education may be necessary in addition to prior work experience.
- At least two (2) years of experience with Medi-Cal managed care is essential.
- Experience with or for a managed health care plan is preferred.
- Familiarity with behavioral health needs and providers is advantageous.
- Proficient in building rapport with members and effectively planning and executing work.
- Ability to articulate ideas clearly and concisely.
- Strong time management skills with the ability to prioritize tasks efficiently.
- Excellent analytical skills are preferred.
- Outstanding communication skills, both verbal and written, with all levels of Blue Shield Promise staff, members, contracted physicians, and participating provider groups.

Pay Range:
The compensation range for this position is: $20.82 to $29.15 for California.

Note:
This range represents the pay scale for this and many other roles at Blue Shield that fall within this pay grade. Salaries are determined based on various factors, including the candidate's experience and location.
