We are seeking a highly skilled Social Services Coordinator to join our team at Blue Shield of California. As a Social Services Coordinator, you will play a vital role in providing telephonic social services support and resource referrals to our members.
Key Responsibilities- Establish Trusting Relationships: Build strong relationships with members, providing person-centered support and services to address their social barriers and care gaps.
- Conduct Behavioral Health Screenings: Conduct telephonic behavioral health screenings and social determinants of health needs assessments with members to identify areas for improvement.
- Provide Care Coordination: Connect members with Health Plan resources and community agencies to ensure they receive the best possible care.
- Document and Track Progress: Document screenings, assessments, referrals, and case notes in electronic case management and tracking systems.
- File Reports: File and follow-up on reports made to government agencies, such as Adult Protective Services and Child Protective Services.
- Provide Post-Discharge Support: Offer telephonic support to members following their discharge from hospitals and other medical facilities.
- Coordinate Services: Coordinate services with state and community programs on behalf of members, particularly when they are unable to do so independently.
- Prepare Reports: Prepare and complete logs and reports as needed for meetings and audits.
- Education: High School Diploma required; Bachelor's Degree in Social Work or equivalent preferred.
- Experience: Minimum of 5 years of experience; minimum two (2) years of experience with Medi-Cal managed care.
- Skills: Broad knowledge of operational systems and practices; excellent analytical, communication, and time management skills.
The pay range for this role is: $20.82 to $29.15 for California.
Please note that this range represents the pay range for this and many other positions at Blue Shield of California that fall into this pay grade. Salaries are based on a variety of factors, including the candidate's experience, location, and current employee salaries for similar roles.
