Job Summary:

At Wellbe Senior Medical, we are dedicated to delivering exceptional care to our patients. As a Patient Care Coordinator, you will be part of a dynamic team that provides compassionate care to the frail population who suffer from chronic and acute illnesses or injuries.

Key Responsibilities:
  • Coordinate care with the medical team, schedule patient appointments, and coordinate diagnostic tests.
  • Act as a community resource for patients, providing support and guidance.
  • Update and maintain digital client records, ensuring accurate and complete information.
  • Manage patient medical records, ensuring compliance with regulations.
  • Provide positive customer service, treating patients and staff with respect.
  • Prioritize urgent patient needs, scheduling needs for Complete Care Assessment, and efforts to engage the unengaged.
  • Collaborate with the interdisciplinary team, participating in regularly scheduled team meetings.
  • Support APC in ongoing panel management and quality of care efforts.
Requirements:
  • High school graduate or GED.
  • MA Certification preferred.
  • 2+ years of experience working for a healthcare practice or hospital.
  • EMR documentation experience preferred.
  • Strong critical thinking skills, strong customer centric focus/service skills, and strong computer skills.
Work Environment:

Ability to lift up to 20lbs, moving, lifting, or transferring of patients may involve lifting of up to 50lbs, and ability to stand for extended periods.

Travel may be required up to 25% locally.
