Job Overview:
The Patient Care Coordination Lead is responsible for overseeing departmental personnel, demonstrating effective leadership by recruiting and developing talent.

This role includes managing the performance of assigned team members, which encompasses recognition, performance assessments, formal coaching, and making informed decisions regarding disciplinary actions.

The position involves making recommendations related to hiring, termination, promotions, and any changes in status for team members.

The Lead exemplifies core leadership qualities and fosters a collaborative team environment. This role requires effective communication with staff to build a cohesive team and align performance objectives.

It is essential to ensure that employees receive the necessary training to fulfill their job responsibilities while adhering to the training plan.

In addition to onboarding, the Lead provides on-the-job training and arranges for additional support when needed. The role includes managing the daily operations of the assigned team, assigning tasks, and offering guidance based on operational directives.

Daily monitoring of staff performance, department staffing, and addressing any issues that hinder goal achievement is crucial, along with providing ongoing feedback to management.

Ensuring compliance with company workflows, procedures, and policies is a key responsibility. The Lead must promptly address any actions or behaviors that deviate from established policies. Identifying recurring issues and reporting them to management based on severity is also part of the role. Recommendations for workflow and procedural enhancements based on observations are encouraged. The Lead assists management in maintaining departmental expenses within budgetary constraints and ensures compliance with all employee-related reporting and tracking.

In collaboration with the Care Coordinator Manager, the Lead will develop, implement, and monitor workflows, programs, and training that enhance the efficiency and effectiveness of care in a Patient-Centered Health Home environment.

Coordination of staff schedules to meet timelines, activities, and milestones is essential. The Lead reviews and maintains effective patient panels for Health Education, Care Coordinators, and Chronic Care Management Specialist teams, collaborating with Health Center Supervisors and Medical/Clinical Directors as necessary.

Support for team members in promoting their roles within Health Centers includes:

Encouraging patient understanding of treatment and care options; Motivating patients to engage proactively with their care teams.

Acting as coordinators for healthcare teams, including facilitating morning meetings.

Assisting patients in accessing resources to overcome barriers to care, such as transportation or childcare.

Aiding patients in managing paperwork and understanding their insurance and coverage options.

Providing assistance to the Manager with programs related to Health Education staff functions, including:
Comprehensive Perinatal Services Program, Title X, Cancer Detection Program, and Family PACT.

Assisting the Manager with programs involving Care Coordinator staff functions, including:

Enhanced Care Management Program and partnerships with relevant organizations regarding care management and addressing care gaps.

Supporting the Manager with programs related to Chronic Care Management Specialist staff functions, including:
Chronic Care Management program and partnerships with external organizations. Ensuring that the ongoing mechanisms for participation meet regulatory and organizational objectives is vital. Documenting program or grant-related activities outside of clinical records as necessary is also required.

Monitoring grant requirements, including coordinating the submission of monthly reports to the Finance and Business Development Departments.

Participation in staff meetings and administering initial and annual skills assessments is expected. In collaboration with the Care Coordinator Manager, the Lead will review improvement plans and training needs for the teams. Other responsibilities may be assigned as needed.

Key Qualifications:

Advanced knowledge and skills are required, including strong written and verbal communication, computational abilities, and technical skills, typically acquired through an Associate's Degree or equivalent experience, along with two years of leadership experience or five years of progressively responsible roles demonstrating leadership capabilities.

A comprehensive understanding of regulatory requirements for relevant programs is necessary to ensure organizational compliance.

Job responsibilities necessitate a deep understanding of the profession to effectively train or guide others and serve as a resource for staff.


Technical Proficiencies:
Proficiency in creating complex documents in Microsoft Word, including tables, charts, and graphs.

Ability to analyze data using Microsoft Excel, employing formulas, functions, and standard spreadsheet elements.

Competence in developing sophisticated presentations in Microsoft PowerPoint, utilizing embedded objects and transitions.

Communication Skills:

The role requires effective verbal and written communication skills to convey opinions and synthesize information.

Resolving conflicts that may arise among employees, clients, or external entities is a key responsibility.

Compiling, analyzing, and preparing information in a clear written format, including reports and documentation, is essential. Effectively communicating technical information to non-technical audiences is also required.

Salary Range:
$67,000.00 - $107,200.00
