Under the supervision of a registered nurse or other licensed independent practitioner, and within the LVN scope of practice, this role is designed to safeguard and enhance health, prevent illness and injury, and alleviate suffering by delivering holistic care to patients and their families served.

QUALIFICATIONS
License /Certification

Required:
CA Licensed Vocational Nurse

BLS
Certification in intravenous therapy and blood withdrawal within six months of hire/transfer.

Education

Preferred:
Associate's Degree in Nursing

Experience

Preferred:
One to three years of experience in outpatient clinic settings (e.g., rural health clinics or federally qualified health centers).

Knowledge/Skills/Abilities
Familiarity with state programs (Family Pact, Every Woman Counts, Child Health and Disability Prevention, Vaccines for Children, etc.)
Ability to communicate effectively and clearly convey changes to staff and providers in a timely manner.

JOB RESPONSIBILITIES
Essential

Oversee daily patient flow.

Conduct regular regulatory checks of the facility in accordance with regulatory agencies (CMS, CDPH, Joint Commission, etc.).

Train and mentor clinical staff on technical skills.


Coordinate and engage in care coordination tasks related to disease management, transitions of care, preventive care, follow-up care, and other focused quality initiatives.

Verify and approve Medical Assistant calculations for medications to be administered by Medical Assistants.

Receive critical lab values via phone, notify the provider immediately, and follow up as per provider instructions.

Order medical supplies, medications, and vaccines from appropriate sources.


Assist and perform procedures in accordance with the guidelines of the LVN Board for the State of California and in compliance with Kaweah Health policy.

Maintain logs and generate reports as assigned (Q/A, Q/C & PI).


Administer medications as directed by the physician, adhering to the guidelines of the California State Board of Nursing and in compliance with Kaweah Health Policy.


Demonstrate the knowledge and skills necessary to provide care and services appropriate to the population served in the assigned unit or work area.

Be knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age-specific needs. Effectively communicate and care for patients and families as reflected in the Plan of Provision of Care.

Perform other duties as assigned.

Pay Range
$30.17 -$45.26
