Key Responsibilities
  • Provide holistic care to patients, including assessing their physical and emotional needs, developing individualized care plans, and implementing evidence-based interventions.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to ensure seamless care coordination and effective communication.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies, procedures, and best practices that enhance patient outcomes and satisfaction.
  • Assist in the management of patient care, including administering medications, performing vital sign checks, and providing basic patient care and treatments.
  • Develop and maintain accurate and comprehensive patient records, ensuring timely and effective communication with healthcare teams and patients.
  • Participate in ongoing education and training, staying up-to-date with the latest clinical practices, technologies, and research in orthopedic and neurological care.
Requirements
  • Current licensure as a Licensed Vocational Nurse in the State of California.
  • Basic Life Support (BLS) certification.
  • Intravenous therapy certification and Blood withdrawal certification within six months of hire/transfer.
  • Preferred: Hospital experience.
  • Testing: For Chronic Disease Management Center: Requires successful completion of the Language Resource Assistant (LRA) test at a General level.
