Job Title: Registered Nurse - Home Health Care

BAYADA Home Health Care is seeking a skilled Registered Nurse to provide high-quality care to clients in their homes. As a Registered Nurse, you will be responsible for delivering patient-centered care, managing client conditions, and collaborating with healthcare teams to ensure optimal outcomes.

Job Responsibilities:
  • Develop and implement personalized care plans in accordance with client needs and preferences.
  • Conduct thorough assessments, identify areas for improvement, and develop strategies to address client concerns.
  • Administer medications, perform wound care, and provide treatments as prescribed by physicians.
  • Monitor client conditions, report changes to clinical teams, and collaborate with healthcare professionals to ensure seamless care transitions.
  • Document client interactions, interventions, and outcomes in a timely and accurate manner.
Requirements:
  • A current license as a Registered Nurse in Pennsylvania.
  • A minimum of one year of recent, verifiable clinical experience.
  • Prior home care experience is strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program.
  • Ability to work independently, manage time effectively, and prioritize tasks.
  • Strong interpersonal and communication skills.
  • Solid computer skills and experience with electronic medical records (EMRs).
