Job Summary:
Liberty Healthcare Management is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Nurse Specialist, you will provide state-of-the-art care and pain management services to our patients and their families.
Key Responsibilities:
- Apply a working knowledge of nursing theories and concepts to deliver high-quality patient care.
- Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure comprehensive care.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure best practices and patient-centered care.
- Collaborate with the Hospice team to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements:
- Current RN licensure in the state of practice.
- Successful completion of a nursing education from an approved school of nursing.
- At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
- Knowledge of the framework, function, and organization of a home health and hospice care program.
- Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
- 12 hours of continuing education annually.
- CPR certification.
