Hospice RN Case Manager Job Description

We are seeking a compassionate and empathetic RN Case Manager to provide direct patient care services to hospice patients and supportive care to their families. As a primary liaison between physicians, patients, and the hospice team, you will empower the patient's caregivers through education and support.

Key Responsibilities:

• Provide direct patient care services to hospice patients
• Supportive care to patients' families
• Liaise between physicians, patients, and the hospice team
• Empower patient caregivers through education and support

Requirements:

• RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
• Current driver's license and automobile insurance
