Job Title: Licensed Practical Nurse

Job Summary:

EverHeart Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families in the comfort of their own homes.

Responsibilities:

  • Conduct home visits to provide hospice services and support to patients and their families.
  • Respond to referral calls and inquiries related to hospice care and its benefits.
  • Obtain necessary orders from the Medical Director and Attending Practitioners.
  • Support with paperwork, such as physician orders and entering data into the EMR.
  • Manage the ordering of medication, DME, and arranging transportation for patients.
  • Collaborate with hospital discharge planners for admission coordination when necessary.
  • Organize and input all essential information into the EMR system.
  • Ensure completion of all required documentation in accordance with Agency policies and regulations.
  • Prepare and finalize paperwork following Agency guidelines.
  • Review new admissions for accuracy.
  • Follow up with patients enrolled in the In Touch Program.
  • Assist with pre-admission tasks, including referrals, medication orders, DME orders, and transport arrangements.
  • Handle referrals and pre-admission communications.
  • Gather patient health information and input necessary data into the EMR.
  • Support with data entry, including physician orders, frequency orders, medication, and DME.
  • Assist with QAPI audits.
  • Supervise the admission binder process.

Qualifications:

  • Graduation from an approved School of Practical Nursing.
  • Current LPN licensure in the State of Ohio.
  • Minimum of one year of experience in Medical/Surgical, Oncology, or community health nursing.
  • Hospice experience preferred.
  • Shall have a valid driver's license.
  • Excellent customer service skills.
  • Detail-orientated with excellent communication, documentation, and organizational skills.
  • Ability to work independently without supervision.
  • Travel to other sites as needed. Travel may be in inclement weather.
  • Ability to lift 25 lbs. of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  • Weekend and Holiday rotation.
