Care Coordinator LVN Job Description

A-Line Staffing Solutions is seeking a skilled Care Coordinator LVN to join our team. As a Care Coordinator LVN, you will play a vital role in determining the needs and eligibility of members for various services through field-based collaboration with medical, behavioral, and social support staff.

Key Responsibilities:
  • Conduct screening and risk assessment interventions per program guidelines.
  • Determine needs and eligibility of members for various services.
  • Collaborate with medical, behavioral, and social support staff to provide comprehensive care.
Performance Expectations:
  • Submit documentation within 3 business days.
  • Timely follow up with members on new service/items requested.
  • 75% travel required.
Requirements:
  • Service Coordination, Case Management, or Nursing experience.
Benefits:
  • Full benefits available after 90 days: Medical, Dental, Vision, Life, Short-term Disability.
  • 401k after 1 year of employment: With employer match and profit sharing.
  • GREAT Hours: Monday through Friday, 40 hours per week, No Weekends.
  • Competitive Pay Rate: $20-$25 per hour.
