Medical Social Worker

We are seeking a highly skilled Medical Social Worker to join our team at WMCHealth. As a Medical Social Worker, you will play a vital role in providing medical social services to patients in the home care setting.
Responsibilities:- Assess patients' social and emotional status using effective interviewing skills.
- Identify client and family needs and priorities of care.
- Recognize and document changes in patients' general physical and mental condition, reporting to physicians and case managers immediately.
- Implement individualized plans of care and discharge plans, coordinating with other disciplines.
- Evaluate outcomes of plans of care and alter plans as needed.
- Set priorities based on patient need and patient response and outcomes.
- Recognize and intervene in situations related to patient safety.
- Adhere to agency and hospital policies and procedures, supporting and participating in changes affecting the department.
- Engage patients and families in treatment plans.
- Mediate between patients, families, and agency staff to develop discharge plans.
- Provide supportive counseling to reduce anxiety and prepare patients and families for discharge and/or referral.
- Counsel patients and/or families regarding relationship problems, adjustment to illness, and improving social functioning.
- Assist clients and/or families to resolve environmental and personal difficulties that impact the success of medical treatment plans.
- Establish linkages with community resources as indicated, such as food stamps, Office of Aging, Meals on Wheels, transportation, personal alert systems, etc.
- Assist clients in accessing federal and state-funded financial entitlements, such as Medicaid, SSI, etc., facilitating application processes by meeting with families and social services, making additional phone calls to research.
- Provide short-term therapy to resolve conflicts related to chronic or terminal illness.
- Counsel for long-range planning, which may include referrals to Hospice, nursing home placement, etc.
- Confer with case managers and other interdisciplinary team members per policy, communicating patient progress toward goals every 2 weeks with appropriate team members 95% of the time, proactively requesting teleconferences to share patient obstacles to goal achievement.
- Record and submit initial psychosocial assessments within two working days.
- Document ongoing evaluations, revised goal setting as needed, and submit documentation per policy.
- Submit activity sheets weekly to facilitate payroll.
- Function and uphold the professional standards of the medical social worker role via evidence-based practice, interactions, and appearance, identifying potential areas of research, helping participate in orientation and precepting of staff as needed.
- Accept accountability for own practice, researching and assisting in the development of staff education tools, identifying potential learning needs of staff.
- Participate in 15 contact hours of education, participating in additional 15 contact hours or 12 academic credits, and sharing knowledge with staff.

Qualifications/Requirements:
- Minimum of two years' experience beyond certification.
- Experience in a mental health or hospital setting.
Licenses/Certifications:
- Licensed Certified Social Worker in New York State.
Other:
- If applicable, the individual performing this job may reasonably anticipate coming into contact with human blood and other potentially infectious materials. Individuals in this position are required to exercise universal precautions, use personal protective equipment and devices, and learn the policies concerning infection control.
- New York State driver's license.
