Key Responsibilities
  • Clinical Skills and Knowledge
    • Conduct thorough assessments of patients' social and emotional status, using effective interviewing skills to identify their needs and priorities.
    • Develop and implement individualized plans of care, working collaboratively with other healthcare professionals to ensure seamless coordination of services.
    • Recognize and document changes in patients' physical and mental condition, reporting to physicians and case managers as needed.
    • Evaluate the effectiveness of care plans and make adjustments as necessary to ensure optimal patient outcomes.
  • Planning and Evaluation
    • Set priorities based on patient need and response to care, ensuring that patients receive the most effective and efficient services possible.
    • Identify and intervene in situations related to patient safety, taking proactive steps to prevent harm and ensure a safe environment.
    • Adhere to agency and hospital policies and procedures, participating in changes that affect the department and supporting special projects.
  • Intervention
    • Engage patients and their families in the treatment planning process, ensuring that they are informed and empowered to make decisions about their care.
    • Mediate between patients, families, and healthcare staff to develop discharge plans that meet patients' needs and ensure a smooth transition to other care settings.
    • Provide supportive counseling to reduce anxiety and prepare patients and families for discharge and/or referral.
    • Counsel patients and families regarding relationship problems, adjustment to illness, and improving social functioning.
    • Assist patients and families in resolving environmental and personal difficulties that impact the success of the medical treatment plan.
    • Establish linkages with community resources, such as food stamps, Office of Aging, Meals on Wheels, transportation, and personal alert systems, to support patients' needs.
    • Assist patients in accessing federal and state-funded financial entitlements, such as Medicaid and SSI, facilitating the application process through meetings with families and social services.
    • Provide short-term therapy to resolve conflict related to chronic or terminal illness.
    • Counsel patients and families for long-range planning, which may include referrals to Hospice, nursing home placement, and other care settings.
    • Confer with case managers and other interdisciplinary team members as needed, communicating patient progress toward goals and requesting teleconferences to share issues and obstacles to goal achievement.
  • Documentation and Reporting
    • Record and submit initial psychosocial assessments within two working days.
    • Document ongoing evaluations, revised goal setting as needed, and submit documentation per policy.
    • Submit activity sheets weekly to facilitate payroll.
  • Professional Development
    • Function and uphold the professional standards of the medical social worker role, adhering to evidence-based practice and interacting with patients and families in a professional manner.
    • Identify potential areas of research and participate in the development of staff education tools.
    • Accept accountability for own practice, researching and assisting in the development of staff education tools, and identifying potential learning needs of staff.
    • Participate in 15 contact hours of education and share knowledge with staff.
