Primary Function:
Accountable for the orchestration of services for children experiencing significant emotional challenges and their families.

1. Facilitate comprehensive service delivery for participants, which includes:

transporting participants as necessary, assembling a child and family team, assessing needs and strengths, developing a personalized Plan of Care (POC), advocating, securing services (including transportation) as required, and providing testimony in court when necessary.

2. Act as a liaison between the child/family and community service providers to ensure the acquisition of necessary services. E

3. Oversee and negotiate the provision of services and agreements through the child and family team, adjusting the POC as necessary and submitting Service Authorization Requests (SAR) when new services are introduced or existing services are terminated. E

4. Prepare, complete, and submit all required documentation by established deadlines, including but not limited to: Progress Notes, POC, Crisis Plans, SAR, measurement instruments, and court-required documents. E

5. Establish and maintain effective communication and collaboration with all stakeholders (e.g., BMCW, probation officers, etc.). E

6. Implement and adhere to the responsibilities of a mandated reporter regarding suspected child abuse in accordance with current policies and procedures, as required. E

7. Participate in all quality assurance/quality improvement studies, meetings, and professional development activities as directed. E

8. Perform additional duties as assigned. NE

9. Contribute as a team player.

Working Conditions:
Work is typically conducted in both field and office settings, with travel required. Local travel is necessary; some state-wide travel may be needed. Flexible hours are required based on program needs.

Minimum Qualifications Required:

Bachelor's Degree from an accredited institution in Social Work or a related field (e.g., psychology, sociology, counseling). A minimum of one year of continuous experience providing mental health services is required.
Bilingual proficiency in Spanish and English is mandatory for bilingual positions.
Able to work and communicate effectively with adults, families, and all team members to implement problem-solving strategies, understand issues, and provide crisis services in an ethical, respectful, and responsible manner.
Strong cultural competency and interpersonal relationship skills are essential.
Excellent communication skills, both written and verbal, tailored to target audiences across various functional areas, categories, levels, linguistic, and economic backgrounds; effective in both team and individual settings.
Demonstrated critical thinking abilities; capable of using logic and reasoning to identify strengths and weaknesses of alternative solutions, conclusions, or approaches to problems and effectively implement strategies.
Highly developed organizational skills with the ability to manage multiple priorities.
Familiarity with Microsoft Office suite is required.
Possession of a valid driver's license, state minimum auto insurance, and adherence to driving standards set by La Causa, Inc. is necessary.
Able to work flexible hours, including evenings and weekends, depending on the needs of the youth/family and program requirements.
Must successfully complete and pass all background checks.
La Causa, Inc. requires all individuals working to be fully vaccinated against Covid-19.
Salary: $38, $48,228.90 Based on Experience
