American Medical Associates is seeking a skilled and experienced MDS Assessor to join our team at our skilled nursing facility in Jersey City, NJ.
About the Role:
The MDS Assessor will be responsible for ensuring the utilization of interdisciplinary rehab team processes in the formulation of MDS/RAI's. This includes observing direct nursing care, reviewing documentation, and making appropriate recommendations. The successful candidate will also assist with chart audits and provide clinical support to the team.
Requirements:
- At least 2 years' experience in a skilled nursing facility working in MDS
- NJ RN license
- Long term care experience
- Knowledge of MDS 3.0
Responsibilities:
- Ensure accurate and timely completion of MDS assessments
- Collaborate with the interdisciplinary team to develop and implement care plans
- Provide education and support to staff on MDS best practices
