MDS Assessor

Recruit Secure is collaborating with a large skilled nursing facility in Jersey City, NJ to find an experienced MDS Assessor. The successful candidate will be responsible for overseeing the patient assessment process, ensuring accuracy and timeliness, and monitoring MDS care plan documentation.
Key Responsibilities:- Monitor and audit clinical records to ensure accuracy and timeliness
- Review documentation and make recommendations
- Assist with chart audits
- Prepare and track electronic paperwork submissions
- Ensure compliance with regulatory requirements and current standards of practice
- Prepare assessment schedules and attend care plan assessment meetings
- Complete required documentation for medical records
- Collaborate with residents, families, and the interdisciplinary team
- Active RN license from NJ
- Experience in long-term care and at least 1 year of job-related experience in the MDS process
- Expertise in MDS 3.0 and PDPM
- Strong experience with Medicaid and Medicare - CMI scoring
- Familiarity with various computer software
- Efficient, organized, detail-oriented, and flexible
- Self-directed but team-oriented
