MDS Assessor

We are seeking an experienced MDS Assessor to join our team at a large Skilled Nursing Facility in Jersey City, NJ. As an MDS Assessor, you will play a critical role in overseeing the patient assessment process and ensuring that assessments and care plans are accurate and timely.
Key Responsibilities- Monitor and audit clinical records to ensure accuracy and timeliness
- Review documentation and make recommendations to improve the MDS care planning process
- Assist with chart audits and prepare electronic paperwork submissions
- Ensure compliance with regulatory requirements and current standards of practice
- Prepare assessment schedules and attend care plan assessment meetings
- Collaborate with residents, families, and the interdisciplinary team to ensure high-quality care
- Active RN license from NJ
- Experience in long-term care and at least 1-year job-related experience in the MDS process
- Expertise working with MDS 3.0 and PDPM
- Strong experience with Medicaid, Medicare - CMI scoring
- Familiarity with various computer software
- Efficient, organized, detail-oriented, and flexible
