As a Clinical Care Coordinator at CommuniCare Health Center, you will play a vital role in navigating the care of complex or high-risk patients. This position requires a licensed LVN Clinical Care Coordinator to work as part of a team, responding to patient calls and providing health education. You will interact and collaborate with multidisciplinary care teams, including providers, nurses, pharmacists, laboratory technologists, health coaches, and dieticians.

This role involves serving as a mentor and role model for Clinical Care Coordinators and other medical staff, providing ongoing guidance and training. Your responsibilities will include coordinating patient care as determined by the provider, working with the provider and clinic staff to identify patients with high-risk diagnoses, and ensuring clinical guidelines are being followed.

You will also work with the Navigation and Innovation Center to answer patient calls regarding clinical questions for clinical care coordination team patients within your scope of practice. This will involve contacting patients directly regarding concerns or clinical questions brought forth by the patient or provider, ensuring that patients are made aware of health issues, concerns, and the way in which they can combat them.

Additionally, you will be available to coordinate resources for high-risk or poor compliance patients as determined by the primary provider. You will collaborate with primary providers and specialty services to optimize patient care and the patient experience. You will also contact CommuniCare post-hospitalization, post-ER, and postoperative patients regarding patient status and schedule appropriate follow-up with CommuniCare.

Other responsibilities include reviewing the no-show list and contacting patients regarding status and clinical plan, assisting in initiating referrals and coordinating transitions of care regarding hospitalization follow-up, palliative care, hospice, etc. You will maintain knowledge of specialty and ancillary provider contract contents, including exclusions and contract terms.

You will also be responsible for entering timely and accurate data into the electronic medical record to communicate patient needs and ensure complete documentation of patient visits and phone calls. Providing exceptional customer service internally and externally is also a key aspect of this role, including greeting each customer with an appropriate introduction, addressing concerns prior to the end of the interaction, and assisting other departments as appropriate.

This position requires a graduate from an accredited school of vocational nursing, a current unrestricted Texas LVN/LPN license, and two years' experience in a primary healthcare setting. Experience with EMR/EHR is preferred, as is the ability to plan, organize, and perform well under stress. Bilingual in English and Spanish is also preferred, as is sensitivity to the needs of the Center's target population. Certification in Basic Life Support (BLS), American Heart Association is required, as is knowledge of personal computers, including internet and intranet navigation.
