Responsibilities

The Patient Care Coordinator - Critical Care will be responsible for:
  • Assessing patients' physical and emotional needs
  • Developing and implementing individualized care plans
  • Collaborating with healthcare teams to ensure comprehensive care
  • Educating patients and families on discharge procedures and care
  • Maintaining accurate clinical documentation

Requirements

To be successful in this role, you will need:
  • A Bachelor's Degree in Nursing
  • Experience in critical care nursing
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Basic Life Support (BLS) certification
