Job Overview

We are seeking a skilled and compassionate Hospice RN to join our team as a Care Coordinator. In this role, you will be responsible for assessing the needs of patients with terminal illnesses and developing comprehensive care plans to improve their quality of life.

About the Role:

  • Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
  • Provide education and support to patients and their families on managing symptoms, pain, and other aspects of end-of-life care.
  • Collaborate with interdisciplinary teams to coordinate care and services, including home health aides, physical therapy, and social work.
  • Ensure timely and accurate documentation of patient information and progress notes.
  • Participate in ongoing evaluation and improvement of care services.

Requirements:

  • Registered Nurse (RN) license in good standing.
  • Bachelor's degree in Nursing or related field preferred.
  • Minimum 1 year experience in hospice or palliative care settings.
  • CPR/BLS certification and valid driver's license required.

Estimated Salary Range: $65,000 - $85,000 per year, depending on experience.
