Patient Advocate Coordinator Job Description
As a Patient Advocate Coordinator, you will play a critical role in ensuring that patients receive the highest quality care and support. Your primary responsibilities will include developing and implementing personalized care plans, coordinating with healthcare professionals to ensure seamless care transitions, and providing emotional support and guidance to patients and their families.

Key Skills and Qualifications
To be successful in this role, you must have a Certified Nursing Assistant (CNA) license and at least six months of patient care experience in hospice, home health, ICU, geriatric, or oncology settings. You should also possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. Additionally, you should be comfortable working in a fast-paced environment and be able to prioritize multiple tasks and responsibilities simultaneously. It would be beneficial if you have experience working with electronic medical records (EMRs) and have knowledge of relevant laws and regulations governing patient care and advocacy.

Salary and Benefits
We offer a competitive salary range of $43,000 - $55,000 per year, based on experience and qualifications. In addition to a weekly pay schedule, our comprehensive benefits package includes health insurance, 401(k) plan, tuition reimbursement, paid time off, and more.
