Compassionate End-of-Life Care Specialist

This position involves overseeing and coordinating the plan of care for a dedicated case load of patients. As a compassionate end-of-life care specialist, you will work with a supportive interdisciplinary team, focusing on holistic patient-centered care and quality at end of life.
Responsibilities include:
- Completing home and facility visits during scheduled hours based on patient and caregiver needs
- Managing comfort measures to ensure quality of life based on the patient's goals
- Coordinating patient care among the hospice team for an assigned case load in a designated territory
- Maintaining accurate, up-to-date documentation based on current CMS guidelines
We are looking for a registered nurse with:
- A current NH RN license
- A minimum of 1-2 years direct care experience required
- End of Life experience preferred
- Excellent communication skills necessary
