The ideal candidate will have a current LPN-LVN license in the state of practice and a minimum of one year's experience in a hospital or clinical setting. Basic computer skills and proficiency in MS Word and Outlook are also required. In this role, you will work with patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge. You will be responsible for building meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

Some details about this position include no dialysis experience required, training may take place in a facility or a training clinic other than your assigned home clinic, and float to various facilities during and after your training. You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays.

The estimated salary for this position is between $65,000-$85,000 per year, based on location and experience.
