We are seeking a highly skilled and compassionate Hospice Care Nurse to join our team in New Brunswick, NJ.
Responsibilities- Conduct thorough assessments of patients' medical conditions, symptoms, and pain management needs in the hospice setting.
- Develop personalized care plans in coordination with the interdisciplinary team and the patient's family.
- Regularly update and review care plans to adapt to changing patient needs and maintain the highest quality of care.
- Monitor patients' vital signs, treatment responses, and overall well-being to ensure optimal care outcomes.
- Collaborate with healthcare professionals to enhance patient comfort and pain management.
- Offer emotional and spiritual support and counseling to both patients and their families during the end-of-life process.
- Respect and honor patients' spiritual beliefs and cultural practices.
- MUST BE AN ACTIVE RN LICENSE IN NJ.
- A minimum of 2-3 years of clinical nursing experience is required, with a preference for backgrounds in hospice, palliative care, or related fields.
- Strong communication and interpersonal skills are essential to engage effectively with patients, families, and the interdisciplinary team.
- An estimated salary range of $45 - $48 hourly.
