Job Summary

This role requires a highly motivated and empathetic RN who can provide direct care to patients and coordinate with the interdisciplinary team to ensure optimal care experiences. As a Case Manager, you will work closely with physicians, social workers, chaplains, and other healthcare professionals to develop and implement individualized plans of care that meet the unique needs of each patient.

Responsibilities
  • Conduct initial and ongoing comprehensive assessments of patients and their families to identify their physical, emotional, and spiritual needs.
  • Develop and implement individualized plans of care that address the patient's medical, social, and emotional needs.
  • Provide education to patients and their families on disease management, medication use, and symptom control.
  • Maintain accurate and up-to-date clinical records, including progress notes, treatment plans, and discharge summaries.
  • Collaborate with the interdisciplinary team to ensure timely and effective communication regarding patient care.
  • Evaluate the effectiveness of the plan of care and make adjustments as needed.
Requirements
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • A minimum of one year of experience as a registered nurse, with a preference for three or more years of experience.
  • Experience in hospice care or a similar setting is preferred.
  • Demonstrated knowledge of the hospice philosophy of care and commitment to providing high-quality patient-centered care.

Estimated Salary Range: $85,000 - $110,000 per year, depending on experience.
