Home Care RN Opportunity

We are seeking a compassionate and skilled Home Care RN to join our team at St. Mary's Home Care and Hospice in Athens, GA. As a Home Care RN, you will provide one-to-one care to patients in their homes, utilizing cutting-edge technology and clinical knowledge to deliver exceptional patient outcomes.

Responsibilities:
  • Assess patient needs and develop personalized care plans
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Utilize Home Care Connect, our virtual care program, to enhance patient engagement and outcomes
Requirements:
  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the State of Georgia
  • One year of experience as a professional acute care nurse
  • Home care experience preferred but not required
