About the Role

As a Home Health Care RN, you will be responsible for client assessment, care planning, intervention, and overall supervision of patient care and outcomes. You will work closely with our interdisciplinary team to provide comprehensive care to patients who require one-to-one attention and monitoring in their homes. Your expertise will be utilized to enhance our clinical excellence with advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs.

Responsibilities

* Provide one-to-one care with patients in their homes
* Utilize cutting-edge technology to deliver exceptional patient outcomes
* Collaborate with our interdisciplinary team to provide comprehensive care
* Develop and implement individualized care plans to meet patient needs
* Monitor patient progress and adjust care plans as needed

Requirements

* Graduate of an approved nursing education program
* Licensure as a Registered Nurse in the State of Georgia
* One (1) year experience as a professional acute care nurse
* Home care experience preferred but not required
