Unity House is seeking a dedicated and compassionate Health Home Harp/Plus Bachelors Level professional to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality care to individuals with complex health needs.
Responsibilities:- Assess health goals and implement a Plan of Care for consumers
- Complete HARP assessments to link members to specialized community-based services
- Provide health literacy education and address social determinants of health to assist members in addressing barriers to healthcare
- Manage chronic behavioral health, medical, and substance abuse conditions for high-risk members
- Link consumers to health and community resources
- Work collaboratively with health providers and other community service providers to promote primary and preventative care
- Assist consumers in reducing or eliminating the need for hospital/ER usage
- Manage all aspects of consumers electronic health records
- Promote a culture of inclusion and belonging
- Bachelor's degree in any field with a minimum of three years' experience in human services
- OR Master's degree in human services and minimum one-year experience working within the field
- OR CASAC and two years of human services experience
- Proficient with Microsoft Office
- Prior experience with Medicaid delivery system preferred but not required
- Must have reliable transportation
- Valid and current New York State Driver License
- Ability to work in a fast-paced environment
